2) With "Show All File Extensions" enabled, I get ".app" on all my
application bundles. So I have to decide which is worse: extensions
on *everything* or extensions on some things. (Notice we get no "Hide
All File Extensions" option?)

Hiding file extensions is very dangerous and I recommed that you never do it.
A lot of Windows trojan horses use that mechanism: they create an
executable and call it, say, "foo.doc.exe". But for users that hide
extensions, it looks like:
foo.doc
and they assume that it is safe.
Always show all extensions. Always.
