About as hard as it is to do it in C.  (Adobe has spent many years  
optimizing Photoshop.)

I don't know that anyone has put much effort into it -- Photoshop  
already has the Photoshop market pretty well tied up.

I did do a layer-based paint program once, and was getting decent  
performance (for a single-process app).  But I never had sufficient  
incentive to really finish it -- it's a LOT of work.
