I had some Windows-only MIDI classes on my open source page for a  
while but they were put aside for refactoring earlier this year. I've  
emailed the archive to Garth already, but if anyone else would like a  
copy feel free to contact me off-list. They work well enough as-is but  
can use some updates to take advantage of newer Rb features. At the  
very least they can be useful as a reference for anyone interested in  
using MIDI on Windows via declares.
